Social Worker - Locality
<p>We are looking for a dedicated and dynamic Locality Social Worker to join our team. The ideal candidate will have a passion for supporting individuals and families within their community, providing comprehensive case management, and working collaboratively with other professionals. This role requires excellent communication skills, a strong understanding of social work principles, and the ability to manage a diverse caseload effectively. Up to £35 per hour dependant on expereince.</p> <p><strong>Job description </strong></p> <p><br /></p> <ul> <li>Assess the needs of individuals and families within the community.</li><li>Develop and implement personalized care plans.</li><li>Monitor and review progress, making adjustments to care plans as necessary.</li><li>Maintain accurate and up-to-date case records.</li><li>Provide emotional and practical support to service users.</li><li>Conduct home visits, assessments, and reviews.</li><li>Facilitate access to resources and services, such as housing, healthcare, and financial assistance.</li><li>Advocate for the rights and needs of service users.</li><li>Work collaboratively with other social workers, healthcare professionals, educators, and community organizations.</li><li>Participate in multidisciplinary meetings to ensure coordinated care.</li><li>Develop and maintain strong relationships with local agencies and service providers.</li></ul> <strong>Qualifications:</strong> <ul> <li><strong>Experience:</strong> Minimum of 2-3 years of experience in social work, preferably within a community or locality setting.</li><li><strong>Education:</strong> Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution.</li><li><strong>Licensure:</strong> Licensed Social Worker (LSW) or equivalent certification as required by state law.</li></ul> <p><br /></p> <p><em>Randstad Care acts as an employment business when supplying temporary staff and as an employment agency when introducing candidates for permanent employment with a client.
